• Name
• Address
• Access Card number

Request a New or Replacement Recreation Access Card

Please complete and submit the “Application/Agreement to Use Association Amenities and Release of Liability” form available through the TownSq WEBSITE and TownSq MOBILE app. If you are a tenant, the property owner MUST sign your request.

Obtain the form using the TownSq MOBILE app:

1. Open the TownSq mobile app and sign in to your account.
2. Tap on Find Notices/Documents.
3. Select Amenity Access.
4. Tap HPWFR.waiver.2.12.24.pdf.
5. Tap Download to save the form to your device.

Obtain the form using the TownSq WEBSITE:

1. Sign in at: https://app.townsq.io/login
2. Click Find Notices/Documents.
   
3. Click on the Amenity Access folder.
   
4. Select HPWFR.waiver.2.12.24.pdf.
